PART 1
Emily is a 32 year old woman. She is a G5 P3. She had a spontaneous vaginal birth of a
4300gm live male 3 hours ago following a precipitous delivery. She received 10u of Oxytocin IM
with the anterior shoulder. She had an intact perineum and felt great after delivery. The baby
went to breast within 30 minutes of delivery.
Upon admission to the postpartum unit her fundus was firm, midline and at umbilicus. She had
moderate to heavy lochia. Vital signs were stable. She had voided just prior to delivery.
She rings the call bell and states she “feels light headed and is sitting in a big puddle of blood”!
Upon entering the room Emily is pale, diaphoretic and anxious.

Your assessment finds that her uterus is boggy, deviated to the right and is three fingers above
the umbilicus. She has not yet voided but states she has not had an urge to “go”. You note a
large amount of bright red lochia has soaked through her pad and onto the bed. You estimate
the blood loss to be approximately 500ml. Her vital signs remain stable.
What nursing interventions would you initiate now? (5 marks)
