OVERVIEW OF CHAPTER LEARNING EXERCISES

Learning Exercise 23.1 has been solved. Although the audit form is simplistic in nature, it demonstrates how a broad goal or service can be broken down into measurable components. In this case, a process audit form has been developed to assess the quality of initial postpartum visits in a visiting nurse clinic. After reading the solution to this exercise, identify process criteria for another goal or service.

Learning Exercise 23.2 provides an opportunity for you to become familiar with the audit process. Do your best to not make this exercise more difficult than it really is.

Discuss your results for Learning Exercise 23.3 with other students who have different levels of work experience than you do.

If you have prior work experience in clinical areas, you might Learning Exercise 23.4 on the easy side. If you have limited previous clinical work, however, you might find your lack of first-hand comparative experiences a challenge.

Learning Exercise 23.5 provides you with information about quality control, as it is important that you understand the many problems inherent in setting standards and determining quality. You should also consider the importance of gathering data and doing research.

You might find Learning Exercise 23.6 very challenging. Although you might be able to state five criteria with relative ease, you might quickly realize how difficult it is to measure many of the things you believe make a difference in quality patient care. As such, challenge yourself to prove or substantiate your own assertions that these criteria truly make a difference in patient outcomes. Also, you might want to discuss with other students the idea of the nursing profession’s need to “prove that they make a difference” or face the threat of being replaced by someone less expensive.

Learning Exercises 23.7, 23.8, and 23.9 ask you to prioritize your obligations to colleagues, patients, employers, and themselves. The political, legal, and ethical risks of a poor decision are significant.

Additional topics to discuss with other students are as follows:

  • Support or oppose the proposition that quality in health care should be quantitatively measurable.
  • Support or oppose the proposition that public and private sector initiatives during the past three decades have been successful in lowering health care costs while maintaining quality.
  • Support or oppose the proposition that traditional natural science study designs, such as the experimental method, are appropriate models for testing hypotheses about quality and health care delivery.
  • Support or oppose the proposition that quality in health care should be measured more by client satisfaction than by traditional outcome measures.
  • Support or oppose the proposition that downsizing (layoffs of professional RNs) and the increased use of unlicensed assistive personnel are currently negatively affecting the quality of patient care.
